The health insurer Helsana once again collected more premiums and gained new customers in 2025. However, the result was negatively impacted by a lower investment performance and a slight deterioration in the combined ratio.

The health insurer was able to increase its premium volume by 8.6% to CHF 8.96 billion in 2025, as can be seen from the insurance group's annual results published on Thursday.

Overall, however, the annual result fell by around a fifth compared to the previous year to CHF 357 million. One of the reasons for this is the performance of the investments, which fell by 1.2 percentage points to 6.9%.

Higher combined ratio

Helsana was satisfied with the underwriting result of CHF 43 million. Overall, however, the Group's combined ratio deteriorated by 1.2 percentage points to 99.5%. A combined ratio of less than 100 percent means that the Group's expenditure in the insurance business was lower than its premium income.

Thanks to cost management, the insurer was able to further improve its efficiency. The operating expense ratio fell by 0.5 percentage points to 6.9 percent.

Helsana was able to slightly increase its equity, which amounted to around CHF 3.3 billion at the end of the year compared to CHF 3.22 billion in the previous year.

More customers gained

In basic insurance, the company is well positioned in terms of premiums for 2026 and has gained 69,000 new policyholders, according to the press release. However, growth was mainly seen in supplementary insurance and corporate business.

In the supplementary insurance business, the combined ratio improved to 92.4 percent. In addition to the individual customer business, the daily sickness benefits business also contributed to this, Helsana reported. The accident insurance business also remained profitable with a ratio of 97.3 percent.

Strategically, Helsana continues to focus on investments in digitalization and new offers. In addition to the further development of its core business, the insurer is planning additional services in areas such as mental health, longevity, fertility and pregnancy, self-determined living and health insurance. In this way, the Group wants to position itself for future changes in the healthcare sector.
